Admissions

Benchmark Transitions™ accepts applications and enrollments on a year-round basis for young adult men and women, ages 18-28.

Our curriculum is designed for students to begin and end at any time throughout the calendar year.

The first step in the admissions process is to review this web site to identify our program as an option for the troubled young adult client.

Next, please request a detailed assessment, by contacting our Admissions department at 800-474-4848. One of our admissions specialists will gladly assist in a full assessment of the prospective student.

Upon completing the assessment by phone, please complete our online application.

Another step is a review of recent clinical reports, such as evaluations and treatment summaries. These documents may be sent via electronic file to admissions@benchmarkyas.com or faxed to us at 909-793-4499.

We may also ask permission to follow-up with a therapist, hospital social worker, psychiatrist or others. A Release of Medical Information is provided below with the Enrollment Forms.

Upon completion of the online application, as well as a confirmation that funding has been secured, we will coordinate a date for prospective family members to tour our campus. This provides an opportunity to meet our staff and other students and tour Educational and Life Skills Center and the State Street Apartments. (Often, families will come for the tour/enrollment at the same time.)

Admission decisions reflect consideration of the individual’s suitability and motivation to commit to a long term transitional living program, as well as their family’s commitment to that same goal.

The following Enrollment Forms will need to be printed out, completed and submitted upon enrollment:

In addition, please bring the following items:

  • Social Security card
  • Certified copy of Birth Certificate (if possible) OR valid Passport
  • Valid Photo ID
  • 30 days of any current prescription medications

*If these items are not available, we will facilitate the student in obtaining them while here at Benchmark Trasnsitions.

We’ve provided a full list of clothing and supplies that will be needed during the student’s stay at Benchmark.
