Admissions
Benchmark Transitions accepts applications and enrollments on a year-round basis. Our curriculum is designed for students to begin and end at any time throughout the calendar year.
You have already completed the first step in the admissions process by reviewing this web site to identify our program as an option for the troubled young adult client.
To request a detailed assessment, please contact our Admissions department at 800-474-4848. One of our admissions specialists will gladly assist in a full assessment of the prospective student.
Upon completing the assessment by phone, please complete our online application.
The following Enrollment Forms will need to be printed out, completed and submitted upon enrollment:
- Admissions Checklist for Parents
- Authorization for Health Care
- Medical History Form
- Medications List
- Professional Release
- Release of Medical Informaton
In addition, please bring one copy of the following:
- Copy of social security card
- Certified copy of birth certificate (if possible)
- Photo ID
We’ve provided a full list of clothing and supplies that will be needed during the student’s stay at Benchmark.
Please review our Location and Directions page prior to making your travel arrangements. (Please note that we have a separate mailing address that is different from our actual physical location.)
